Process accidents are divided into two levels of plant and workshop management.

Third, the content of plant-level process accidents

3. Man-made accidents that seriously affect the balanced production of ball mills and the deterioration of flotation indicators. The intermediate control indicator did not meet the technical card requirements and exceeded the allowable range.

4. Improper operation of the mine and grinding, resulting in self-control disorder or other equipment failure, resulting in parking for more than four hours.

5. In the case of stable nature of the mine, the on-duty personnel did not operate carefully, resulting in a grade or recovery rate of more than 5% of the concentrate grade or more than the planned indicator, and the ratio of mutual ratio plans increased by more than 3%.

6. Unauthorized changes to the technical conditions specified in the technical operation card, and exceeding the allowable range, resulting in a decline in concentrate grade and recovery rate of more than 5%, and an increase of 3% or more.

7, perversion production process, and the process failed to find run, run, leak, drip, blocking, etc., metal induced accidents or loss of copper, zinc product confusion.

9. Falsification and fraud, resulting in statistical work and metal balance work can not be carried out normally, resulting in substantial loss of behavior.

10. The solid content of the thickener overflow water exceeds 4kg/m3 for four consecutive hours. (tentative)

11. Improper operation causes the thickener to collapse, which in turn affects the entire production.

12. The water isolation pump and filter press cannot be operated correctly, causing the whole plant to stop. The concentrate water is seriously exceeded, and the tailings conveying pipeline is seriously blocked.

13. Improper maintenance caused the tailings dam to partially collapse.

14. The nature of the ore was abrupt, and it was notified that the process conditions were not adjusted in time, which caused the indicator to deteriorate. Problems occurred in the post, and the dispatch and related responsible persons were not notified in time, resulting in the next process being affected for more than 4 hours.

15. The handover class did not abide by the handover system, malicious operation, and artificially disrupted the stability of the process, which led to the deterioration of the next class of indicators.

Fourth, workshop process accidents

Similar to the factory-level accident, but the plot is relatively light, the damage is small, and the short-term impact can be used as the content of the workshop process discipline management. Each workshop can make corresponding rules according to the management characteristics of the workshop.

V. Notification of process accidents

1. The factory-level process accident must fill out the “Process Accident Report Form” to the factory office within three days. The factory office reports to the company in charge of the leadership and relevant departments. The workshop-level process accident must hold an accident analysis meeting on the next day of the accident. Relevant personnel from the room and workshop participated in the project, and put forward handling opinions and improvement remedial measures, which were approved by the director's office. On the same day, the "Process Accident Treatment Opinion" was issued.

2. Due to equipment accidents caused by illegal operation, it shall be implemented in accordance with relevant regulations of the company.

6. Reward and punishment of relevant personnel

1. Responsible persons shall be given fines, notices of criticism, warnings, demerits, delistings, etc. depending on the severity of the circumstances. The company's office will approve the implementation or report, and the above major accidents will be reported to the company for approval.

2. For the rewards of the personnel who have prevented the accident from happening or remediating the accident, the unit will give an opinion, and will be rewarded after being reported to the company in charge of the leadership of the company.
